I am not getting friend request even though I have the setting set to "Everyone"

Hi,
I am not getting my friend's request on my Facebook account. I had checked that the earlier setting was - "Friends of friends" for who can send me Friend request and so she didnt see "Add as friend" button on my profile. Then I changed the setting to "Everyone" for who can send me friend requests and then she saw the button and could send me the friend request. At her end it shows "Friend request sent" however at my end, however I did not receive any friend request on my profile nor did I receive any email from Facebook regarding any friend request.

Please let me know what should I do in such case ?
Is there any other setting preventing it to appear on my page and accept her request ?
