Someone created a username for my account without my consent. How can I remove it? And how can I give a final & permanent username for my Facebook Account? I really expect that you guys may really help out me FacebookForums. Tell me the way though I'll be grateful to FBForum.

You can change your username once; however after that you will not be able to.
To change your username go to the Settings tab of the Account Settings page. Scroll down to "Username" and click "change."
